General G4 Implant Solution

We are unlike any Dental Office you may have visited. Our Doctors and the entire staff are welcoming, caring, understanding, non-judgmental, easy-going, professional, and extremely friendly. We will treat you like a member of our family and change your perceptions of the Dental Office.

The entire procedure is completed in a 24 hour period. 

Day 1: Consultation- Ct Scans, X-Rays, measurements, pick tooth shade and shape, facial scans, discuss outcome with our Doctors, etc…….

Day 2: Surgery- Any extractions, placement of the implants (you will be required to go without teeth overnight for the last time in your life!. Remember, we are not giving you a temporary converted denture. You are receiving a custom-milled, highly aesthetic, hybrid bridge with a one piece milled titanium bar!)

Day 3: Delivery of PERMANENT hybrid milled bridges


The ALL-ON-4® Treatment Concept calls for several visits prior to your actual surgery. At some point either their in-house denture lab or an outside lab will fabricate a denture for you. Following your implant placement, that denture will be converted to fit on your implants. Your models will then be sent to an outside lab or milling center for fabrication of a titanium bar, you will return for try-in’s several times within the next several months, until finally your permanents (same denture, with a titanium bar) are ready. This process generally takes 6 months. It has nothing to do with healing, because the titanium bar actually adds stability to the implants. It has to do with capabilities.

Not necessarily, whether you have a restoration on 3,4,5 or 6 implants if one of these implants fails the restoration is compromised. This myth is perpetuated by Dentists with no experience in the G4 Implant Solution, or for other motives.

The fact is that when placing 6 implants, sinus lifts and/or bone grafting are usually necessary (these procedures lead to higher failure rates, increased healing times, and dramatically increased costs). Having more implants in the mouth also reduces your natural bone area. With the Golpa G4 Implant Solution the rear implants are placed at approximately 45 degree angles, avoiding the sinus and engaging more bone. The fact is that the G4 Implant Solution has the same or better success rate as traditional implants

If our Doctors, after examination, determine that more than 4 implants would be beneficial and feasible without additional procedures in your situation, they will place them.

Initially, some patients find the experience of having their new bridges for the first time to be challenging. Initially, the mouth thinks this is a foreign object, and cannot duplicate the exact feel and function of your natural teeth, especially if your teeth have been missing or compromised for many years. It will be a new sensation that will take you a period of time to adjust to. Given time the bridges will feel as close to your natural teeth as possible.

Corporate dental centers are based on meeting the needs of the shareholders, not the needs of the Patients. No, Dr. Golpa, and his staff are EXPERTS; When we use the word expert, we mean that the person has excelled in his chosen field of work or profession.
Look at this example. A famous comedian married 6 times and called himself a specialist in marital relations. True, he is specialist in marital relations, but is he an expert? No.
It becomes clear then that one can become a specialist in his chosen field of study or work by passing certifications and getting degrees, but he does not qualify as an expert until he is referred to as such by his clients. Thus specialist is a tangible asset in the form of a certification or degree, while expert is an intangible asset that a person EARNS by showing or displaying his expertise to his clients.
This procedure is not taught in any formal Dental School, and is a combination of all processes. The technique is learned through advanced training and EXPERIENCE. We bill all insurance companies as a courtesy for you. Typically insurance companies will only cover the extractions necessary @ $300-$500 per extraction ( this is not guaranteed). Any refund will be paid to you 30 to 60 days following the procedure.

In some cases medical insurance may cover more of the procedure, but only in case of an accident. If you have any questions contact your insurance provider and ask them to clarify your coverage’s,

Payment in full is due at the time of surgery. When using any financing, all arrangements must be made 2 weeks prior to surgery.

Yes, Funds you withdraw from your HSA are tax-free when used to pay for qualified medical expenses as described in Section 213(d) of the Internal Revenue Service Tax Code. The expenses must be primarily to alleviate or prevent a physical or mental defect or illness, including dental and vision. A list of these expenses is available on the IRS Web site, www.irs.gov in IRS Publication 502, “Medical and Dental Expenses.”

Any funds you withdraw for non-qualified medical expenses will be taxed at your income tax rate plus 20% tax penalty if you’re under 65.

Generally, taking money out of your Individual Retirement Account before turning 59 1/2 is a no-no because of the 10 percent early withdrawal penalty. However, if you have medical expenses for the year that would be deductible if you itemize your deductions, you can exempt that portion of the early IRA distribution from the penalty. Qualifying medical expenses include those you pay for medical, dental and vision care for yourself, your spouse and your family. Claiming the exception saves you the 10 percent early withdrawal penalty, so not reporting it properly can be a costly mistake.

If, for a taxable year, you itemize your deductions on Form 1040, Schedule A (PDF), you may be able to deduct expenses you paid that year for medical and dental care for yourself, your spouse, and your dependents. For years beginning after December 31, 2012, you may deduct only the amount by which your total medical expenses exceed 10% of your adjusted gross income or 7.5% if you or your spouse is 65 or older. Because we have Patients visiting world-wide, we are usually booked 6-10 weeks out. We do have an occasional reschedule, but this is rare. If you are seriously considering the G4 Implant Procedure, we encourage you to secure your surgery date as soon as possible.

Yes, a $5,000 is necessary to schedule your surgery date. When you schedule your surgery date, we are committing significant time and resources to you, as well as, tying up the schedule for other Patients.

It is important that you are committed to having the procedure done prior to scheduling your appointment.

If it becomes necessary for you to cancel your surgery 2 weeks or greater of surgery, a cancellation fee of $500.00 will be charged.

If it becomes necessary for you to cancel your surgery 2 weeks or less before your surgery, 75% of the deposit will be forfeited.

CHANGE OF SURGERY DATE: If you request us to change your surgery date, it makes a tremendous amount of work for our staff and inconvenience to other patients who must adjust their times.

If your request to change dates is made more than 2 weeks before the date we have reserved for you, we will try to accommodate your request with no penalty.

If you call us less then 2 weeks before your surgery date, there will be an additional rescheduling deposit of $500.00 that must be paid before we can reschedule your surgery.

A deposit is required before scheduling any surgery. The deposit will be $5000.00. The deposit is credited to the amount of your surgery.

If we are unable to perform your surgery for reasons beyond your control a full refund of the deposit will be issued.

Surgical Questions

Because you are prescribed medication 1 week prior to surgery, and the expertise level of our Doctor’s, Patients report that there is little pain regardless of the number of extractions, only moderate soreness. Any discomfort will generally subside in 72 hours or less. Flying or moderate activity is Ok 24 hours after surgery.

Our licensed Anesthesiologists will administer and monitor your conscious sedation. Conscious is a combination of medicines to help you relax (a sedative) and to block pain (an anesthetic) during a medical or dental procedure. You will remain in a “twilight” state, and can respond to commands, but remember little of the procedure. Conscious sedation lets you feel no pain, recover quickly, and return to your everyday activities soon after your procedure.

  • Antibiotics:
    Amoxicillin (Amoxil, Trimox) 500mg 28 tabs
    Take one tab three times per day starting two days before surgery until finished.

If allergic to Penicillin (Non-Anaphylactic Reaction)
Cefaclor (Ceclor, second generation Cephalosporin) 500mg 22 tabs
Take one tab three times per day starting two days before surgery until finished.

If allergic to penicillin (Anaphylactic Reaction: hives, itching, angioedema, respiratory distress)

  • Anti-inflammatory:
    Decadron(Dexamethasone) 4.0mg-5 tabs Contraindicated for diabetic patients

Diabetic Patients will be prescribed:
Motrin(Ibuprofen) 600mg 10 tablets-Take one tab one hour before surgery and then one tab three times a day(1 every 8 hours) until gone.

  • Peridex (Chlorhexdine Gluconate) 16 ounce bottle-Rinse capful twice a day for 30 seconds. Start one week before surgery and resume after surgery for one more week.

The surgical portion of the procedure is approximately 1 hour per arch (upper/lower)

We ask that you quit smoking prior to your surgery. Cigarette smoking has long been known to be associated with a variety of oral conditions including periodontal disease, bone loss, tissue loss, tooth loss, edentulism, peri-implantitis and dental implant failure. Nicotine in tobacco has been shown to reduce the blood flow in the mouth. Pipe smoking can be worse than cigarettes due to the higher temperatures generated in the upper jaw. Smoking detrimental to implants,You will be required to sign a waiver prior to surgery. Our Doctor’s prefer to do all extractions (the day of surgery) in our office. It is important that the teeth are extracted carefully in order to preserve bone. If care is not taken, because another Dentist plans to put you in a denture after extractions, bone structure can be compromised. It is also beneficial to the Patient to have extractions under sedation (Sleep Dentistry).

This is the least invasive procedure which is done under sedation. After extraction and implant placement, the bridge will be sitting on implants only and NOT on the soft tissue. The gum will be completely closed up after surgery therefore there is no socket exposed and no dry socket.. Patients are placed on anti- inflammatory medication prior to surgery, therefore there is no pain or swelling. 9 out of 10 patients do not even take any pain medication after this procedure.

Natural Translucency

Whereas other ceramics must be painted/shaded to “appear” translucent, Ultra truly is a translucent material ideal for cosmetic dentistry.

SUMMARY: Crystal Ultra is by far the strongest esthetic ceramic material, higher than 3M Lava Ultimate, and higher than e.max, vita mark II and empress CAD, making Crystal Ultra the toughest dental material in use for cosmetic, minimally invasive and implant dentistry.

SCIENCE: The higher compressive strength of Crystal Ultra is the result of special manufacturing processes used to bond the glass and polymer materials, including the use of a special sylanated glass ceramic which chemically bonds to the polymers, as compared to other hybrid or ceramic materials that form no such chemical bond.

Compressive strength, which best simulates chewing forces in the mouth, is measured by applying both upward and downward pressure on an object until it breaks. Unlike Flexural Strength, which similates breaking a stick over your knee and is measured many different ways according to different international standards, compressive strength is measured only one way according to one ISO standard.

Zirconia is the result of a chemical process: the basis is zirconia sand, partially stabilized with yttrium and made into Zirconia blanks through a mechanical process. Among the dental ceramics available today, zirconium oxide is without any doubt the material with the highest flexural and tensile strength. By omitting the ceramic layering, a more massive structure can be created, resulting in an overall higher resistance of over 200%. In this way, ceramic chipping and abrasion to the antagonist are being avoided.

Prettau® Zirconia is partially stabilized with yttrium and enriched with aluminium. This results in positive material properties like a high bending strength of up to 1,570 MPa (Study of the University of Munich, Dr. Beuer, February 2007), a high temperature resistance (of up to 2,600°C), as well as a constant shrinking factor granting the highest possible precision.

Lifetime Dental Implant Warranty & 5 Year Prosthetic Warranty

Many practices offer some warranty as a courtesy, but we put in in writing. We stand by our G4 Implant Solution with a Lifetime warranty on our Dental Implants. Most practices will charge extra for these services, even though the cause of this failure is sometimes due to their own error.

In the rare case of implant failure, not only we remove the implant and replace it at no charge, but any corrective procedures required will also be performed at no cost.

Your bridges and implants should receive special care and maintenance every 6 months. If you choose to participate in our voluntary Maintenance and Warranty Program a complete assessment of your dental implants and bridge(s) will be conducted at each re-care appointment which will include:

Visual assessment of the patient; including examining the patient for signs of inflammation, bleeding, changes in probing depths, mobility, or exudate around the implant(s), and debridement of the your dental prosthesis. In addition, we will radio-graphically examine your Implant(s) to determine whether changes have occurred in bone levels. We will also conduct a careful review of your current medications, as well as a review of your health status.

Due to the nature of the tissue attachment surrounding the implant(s), Dental Implants may be more susceptible to inflammation and bone loss when plaque accumulates. Proper maintenance of the tissues will ensure longevity of the Implant(s). Your home-care routine can significantly influence Implant success. After your re-care appointment, a patient-specific home-care routine is developed for you.

Your Crystal Ultra®/Titanium bridge(s) are warrantied for a period of 5 years. Within the warranty period we will repair or replace bridges that fail due to workmanship at no charge. This period can be extended through our Maintenance and Extended Warranty Program.

Your Prettau Zirconia bridge(s) are warrantied for 10 years. Within the warranty period we will repair or replace bridges that fail due to breakage at no charge. This period can be extended through our Maintenance and Extended Warranty Program.

Dental Implant & Prosthetic Conditions and Limitations

If a dental implant fails to integrate we will replace it at no cost, one time only. An annual implant examination including x-rays and regular cleanings is an absolute requirement for any warranty to apply. The Lifetime Warranty Dental Implant Warranty does not apply to continue to smoke or persons who have, or who develop, an uncontrolled systemic health problem, such as, but not limited to; diabetes, autoimmune disorders, severe osteoporosis, or any condition requiring long steroid use. If smoking or a medical condition is suspected as the cause of implant failure or other complication that requires treatment, an examination and blood tests will be required for warranties to apply.

Crystal Ultra®/Titanium bridges are warrantied for 5, years but can last much longer with proper care and maintenance .

Your Crystal Ultra®/Titanium bridge(s) are warrantied for a period of 5 years. Within the warranty period we will repair or replace bridges that fail due to workmanship at no charge. This period can be extended through our Maintenance and Extended Warranty Program.

Prettau Zirconia® Bridges are warrantied for 10 years, but can last much longer with proper care and maintenance .

Aftercare Questions

The bridges should be cleaned every 6 months. Proper maintenance is extremely important to the stability of the implants, as well as, the longevity of the bridge. We do provide an optional Maintenance Program that can help in achieving the maximum results at reduced fees, and provide valuable piece-of-mind. The cleaning can be done by a local affiliate Dentist, or any dentist familiar with the All-on-4 Procedure.

Brush your bridges like you did your natural teeth. Toothbrush and toothpaste, 3 times a day; 2 minutes total brushing time. Electronic toothbrush is highly recommended.
Waterpik after 1 wk of receiving your bridges. During those 7 days without your waterpik, you will use the rotary brush that were provided by our office to clean under your bridges. Dip the proxy brush in your prescription rinse instead of using toothpaste on it. After a week you may start using your waterpik. Use your waterpik after each meal if possible, and if not; be sure to use it in the morning, and before you go to bed at night. Keep in mind that if you do not waterpik daily, food will accumulate under your bridges, causing plaque build up and tartar on your implant abutments. A portable waterpik is recommended, so you can easily travel with it on the go.

In the rare case of an infection, excessive swelling, pain etc., a prescription will be called into your local pharmacy. The only way for any Dentist to treat these symptoms is via medicines.

Since you are receiving a permanent set of teeth, any issues with the bridge will be negated.

We recommend returning in 4 – 6 months for a reline of your bridges, and to verify the integration of your implants. At this time we will make changes to your bridges if you wish.

There will be a very small space between your bridges and your gums. This space is to allow you to clean this area with a Waterpik. After extractions your gums may recede, this recession period is usually between 4-6 months. After your gums have fully receded a reline of your bridge will be necessary. We will add material to your bridge to ensure an optimum fit.

Waterpik Water Flossers provide a unique combination of water pressure and pulsations to massage and stimulate gums, and remove plaque deep between teeth and below the gumline where traditional brushing and flossing can’t reach.

The Waterpik Ultra Water Flosser is ideal for those who don’t like to floss, and anyone with braces, implants, crowns, bridges or periodontal pockets.

Yes, there is a small space between your bridges and your gums, because your bridges are implant supported and sit on the abutments. You can easily use a Waterpik to clean this space at least twice a day.

All cases are different depending on bone density, and the level of torque our Doctor’s are able to achieve on the implants.
